Primary Focus Intake Questionaire
This does not replace the Intake/discovery/exploration session, but rather helps stir up the client's thinking and how they look at themselves and their lives. You can use this information later to check up on their feeling of progress, or as a resource for other coaching. |
Release Authorization
This gives you permission to log hours and use their name for credentialing purposes. Their personal information will not be given out, but you need to provide this info to CTI or the ICF in order to get your letters. |

Guideline of Intake Questions
Remember, the Intake session is really about "Designing the Alliance" and "Creating the Relationship". Everything that you do (values mining, fear exploration, Future Self, Saboteur) is all about setting the groundwork of your coaching relationship. Use this as a guideline, but feel free to "Dance in the Moment". |
